AFFIDAVIT OF WITNESS

I, Joel Gebusion, of legal age, single and a resident of Talamban, Cebu

City, after having sworn in accordance with law, do hereby depose and
state THAT:

1. That I am a licensed clinical psychologist and provide consultation

and therapy at the Center For Treatment, Cebu City and am a
practicing psychologist for the past 15 years.

2. That I personally know the victim, Ronith Boc since she is my

patient and we had therapies on her psychological disorder.

3. That I personally know that the victim, Ronith Boc has tendencies
to inflict self-injury due to her psychological disorder.

4. That I personally know the accused, Roque Dan Carijutan because

Roque Dan approached me concerning the certainty of the victim,
Ronith Boc’s psychological disorder.

I executed this affidavit to attest the truthfulness of the foregoing facts

and to support the filing of Criminal Cases for Roque Dan Carijutan for
violation of RA 9262 Section 5a.

IN WITNESS WHEREOF, I hereunto affixed my signature this 18th
