CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

The ST. SCHOLASTICA VILLAGE CHAPEL YOUTH CHOIR, herein represented by GILES

EDGAR B. DAYA, Musical Director and Project Manager of “Musika Ng Kabataan, Tungo Sa

Kaunlaran”, hereinafter referred to as the “First Party:”

And

LUCENCIO Y. LETABA, of legal age, Filipino, and a resident of San Jose Tacloban City,

hereinafter referred to as the “Second Party”

WITNESSETH

1. That the First Party has signified his intention to acquire the services of the Second

Party, to which the latter has accepted to provide;

2. That the Second Party hereby possesses the training, experience and skills required to

perform the job as described herein;

3. That in view hereof, the Second Party is hereby contracted as Facilitator for the Song

Writing and Arranging Workshop held last December 17 and 22, 2017 at the St.

Scholastica Village Chapel Guindapunan, Palo, Leyte, and Consultant for the “Musika ng

Kabataan, Tungo Sa Kaunlaran Project, at the rate of TEN THOUSAND PESOS

(P10,000.00) which will be given once the project is completed.

4. That as Facilitator/ Consultant, the Second Party is expected to render the following

functions and services:

a.) Act as Facilitator for the Song Writing and Arranging Workshop;

b.) Provide Consultancy Services for the following:

1. Pre- Concert Stage- mounting and technical rehearsals, audio and technical

needs, visuals and concert repertoire;

2. Concert stage- refinements in arrangements and in live audio mixing;

 3. Recording stage- only with respect to arrangement ideas and mixing.

c.) And such other artistic concerns relative to the implementation of the project.

5. That it is understood that this contract does not create an employer-employee

relationship between the First Party and the Second Party.
