SCHOOL FIELD TRIP -PERMISSION SLIP

Your child has the opportunity to tour the Reginald F. Lewis Museum in celebration of Black History Month. Students
will learn about prominent African American Marylanders. Your son/daughter will get to view the three permanent
exhibits as well as multiple special exhibits being shown in honor of Black History Month.

call their office at Tel: 443-263-1800. Review and sign this permission slip and complete all required forms and return
them by Thursday, February 22nd, 2018 so that your child does not miss out on this amazing and memorable experience.

I give permission for my child to attend the Reginald F. Lewis African American Museum field trip with his/her
teacher Friday, February 23rd, 2018. I can be reached at: _____________________________ during the field trip.

I understand that the trip is school sponsored and that students will be supervised by schools personnel. I understand
that even though the trip is related to a school activity, participation is NOT REQUIRED. My child and I have fully read
this permission slip and all of the Parental Packet documents and understand the arrangements for the trip to Reginald
F. Lewis Museum. Furthermore, I understand that all necessary precautions and plans for the care and supervision of
students during the trip will be taken and recognize that while my child is participating in this event that he/she must
adhere to Baltimore City Public School System’s, Friendship Academy of Engineering and Technology and Reginald F.
Lewis Museum rules and regulations. I fully understand that by signing below, that since student participation is
voluntary; the school, the school supervising personnel and chaperones will assume no liability for loss or injury that
might result from an accident and that failure by my child to adhere to the rules and regulations will result in disciplinary
action or removal.
