PYSCHIATRIC REPORT

Name of Patient: Lucky M. Lobo Attending: Dr. Frank S. Stein


Age: 20 years old Date Admitted: 3/3/2009

Identifying Information:

- Patient is identified as Mr. Lucky M. Lobo, currently 20 years of age, male, and a
resident Manila.
- Currently living with an older brother who facilitated his admittance in the hospital.

Personal History/Background:

- Mr. Lobo was born on October 2, 1988 and is the second son of Mr. Pedro Lobo Sr. and
Mrs. Norma Lobo
- Both parents are listed as deceased
- The father died from brain cancer a few weeks after Mr. Lobo was born, while the
mother died after both children were removed from her custody, with the cause of her
death unknown
- Mr. Lobo has one older brother, Mr. Pedro M. Lobo Jr., who has no history of any mental
illness or problem
- Mr. Lobo was a victim of child abuse at the hands of his own mother
- A criminal case was filed against the mother after it was found out that she was
physically, verbally, and mentally abusing Mr. Lobo
- The mother eventually lost custody of both children, and both children were sent to a
close relative

Circumstance/s Leading to Admission:

- On March 1, 2009, Mr. Lobo was reported to have attempted to attack one of their
neighbours, an old woman, after the latter started shouting at Mr. Lobo for some
unknown reason. Luckily, the older brother was present at the scene and was able to
prevent Mr. Lobo from attacking the old woman. During that moment, Mr. Lobo was
reported to show hyper-aggressive and violent behaviour, which is far from his usual
docile and quiet nature. After he was restrained, Mr. Lobo passed and when he woke up,
it was said that he did not remember the past events that transpired.
- The older brother reported that Mr. Lobo has not exhibited any violent or aggressive
behaviour prior to the above-stated incident. However, it was also reported that Mr. Lobo
is known to grow hysterical and afraid due to an unseen and unknown figure coming out
to get him. This type of behaviour of Mr. Lobo mostly manifest when being chastised or
scolded. The older brother believes that it is the lingering manifestation of the abuse that
Mr. Lobo suffered from his mother.

Brief Hospital Course:

- Mr. Lobo is a very quiet and well mannered young man


- He is generally shy and would not speak until spoken to
- He has shown to have above-average intellect and is fully capable of all his motor skills
- He told me that he occasionally suffers cases of “black-outs”, wherein he loses
consciousness and does not remember anything that transpired in between.
- He also told me that he occasionally see their “mother” shouting at him and threatening
to hurt him

Diagnosis:

Primary – Schizophrenia, paranoid subtype

Secondary – Possible Post-traumatic stress disorder, dissociative disorder

Treatment Plan:

Due to Mr. Lobo’s serious diagnosis, a tendency of violence towards himself and others, and
lack of effective medication, he is deemed admitted as an in-patient at the hospital. He will be
placed on a combination of medicine cocktails and electroconvulsive therapy, placed on the
long term treatment unit indefinitely.
