AFFIDAVIT

I, DANJIE T. CINCO, of legal age, Filipino, and a resident of Mangagoy,

Bislig City, Surigao del Sur, after being duly sworn to in accordance with law,
hereby depose and say, that:

1. I am a member of Philippine National Police, assigned at Butuan City

Public Safety Company as Police Officer I;

2. On August 24, 2017 at about two o’clock in the afternoon, the

undersigned and PO1 Jake Mark. E. Corminal were reprimanded by PCI
Christian Rafols II due to our demerits;

3. We were tasked to clean the BCPSC Office Comfort Room, stocked the
confiscated mufflers inside the BCPSC Office and even cleaned the
canals;

4. The total duration of the tasks we have done was for almost four (4)
hours;

5. This Affidavit is being executed to attest to the foregoing facts.
