legal age, Filipinos and residents of Dipolog City, Zamboanga del Norte
after being duly sworn to in accordance with law, depose and state THAT:

1. We are the surviving children of Marcelina Molina – Antisoda who

died on December 17, 2005 in Dipolog City and her deceased
spouse Jose Antisoda.

2. Our parents were survived by the following children/heirs:
