AFFIDAVIT OF DISCREPANCY

That I, PROCESO B. GARCIA of legal age, married, Filipino

citizen with residence at Purok 2, National Highway, Bonfal Proper,
Bayombong, Nueva Vizcaya, after having been duly sworn to in
accordance with law hereby depose and state:

1. That I was born to on July 16, 1953 in Bgry. Uddiawan,

Solano, Nueva Vizcaya to spouses PATERNO A. GARCIA
and JUVENCIA F. BATOON;

AFFIDAVIT OF DISCREPANCY

That I, PROCESO B. GARCIA of legal age, married, Filipino citizen

with residence at Purok 2, National Highway, Bonfal Proper, Bayombong,
Nueva Vizcaya, after having been duly sworn to in accordance with law
hereby depose and state:

1. That I was born to on July 16, 1953 in Bgry. Uddiawan, Solano,

Nueva Vizcaya to spouses PATERNO A. GARCIA and JUVENCIA F.
BATOON;

2. That the facts and circumstances surrounding my birth were duly

reported and registered at the Office of the Local Civil Registrar of Solano,
Nueva Vizcaya and at the National Statistics Office (NSO) Bayombong,
Nueva Vizcaya where I was registered by the name PROCESO B.
GARCIA;

3. That however, when I was a kid, my parents told me that my name

is CARMELO B. GARCIA. For that reason, I have been using CARMELO B.
GARCIA as my name in all my documents and other pertinent papers;

4. That the names PROCESO B. GARCIA and CARMELO B.

GARCIA refer to one and the same person, as referring to myself;

5. That I am executing this affidavit for the purpose of confirming the

truth of the foregoing facts and for any other purpose which it may legally
serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this
