affidavit of discrepancy

I, ERLINA HITGANO VILLEGAS-BACUS, Filipino of legal age, married and a

resident of Purok Lacharon, San Pedro District, Pagadian City, after having duly
sworn to in accordance with law, do hereby depose and say that;
I am daughter of spouses Matias Villegas and Paterna Hitgano Villegas;
I was born on January 21, 1974, at Pagadian City, Zamboanga del Sur;
I am married to Marlowe Bacus;
However, as appeared in my Certificate of Marriage, the middle initial and
maiden name of my mother were erroneously entered as PATERNA I. GITGANO,
when in truth and in fact, it should have been PATERNA E. HITGANO, as the true
and correct entry;
This erroneous entry, if not corrected in time will cause confusion as to my
persona, official and future transaction;
