PETITIONER, unto this Honorable Court, most respectfully files this

application for the issuance of her commission as Notary Public for the province of
Cagayan and in the City of Tuguegarao.

In support to this petition, the undersigned states that:

1. She is a Filipino Citizen, married, over twenty one (21) years old, residing at
Corners Cusipag and Langcay Streets, Zone 3, Linao East, Tuguegarao City.
Copies of the picture of the petitioner with signature are hereto attached
marked as Exhibits “A” , “A-1”,“A-2”;

2. She finished her law degree at the Philippine Law School, Pasay City, Metro
Manila;

3. She was admitted to the Philippine Bar on September 14, 2017, a copy of
certificate of admission and rating hereto attached, marked as Exhibits “B”
and “B-1” respectively;

4. She is a bona fide member in good standing of the Integrated Bar of the
Philippines, Cagayan Chapter, copy of her certificate of good standing marked
as Exhibit “C”;

5. She has paid membership dues as evidenced by the certification issued by the
Integrated Bar of the Philippines, Cagayan Chapter, hereto attached as Exhibit
“D”;
 6. She has no pending administrative case as evidenced by the certification issued
by the Office of the BAR Confidant and the Integrated Bar of the Philippines ,
hereto attached as Exhibits “E” and “E-1”;

7. She has her law office at her residence at Corners Cusipag and Langcay
Streets, Zone 3, Linao East, Tuguegarao City, sketch of the Office Location is
hereto attached as Exhibit “F”;

8. She has paid her Professional Tax for the fiscal year 2014 under Professional
Tax Receipt No. __________ , a copy of which is , hereto attached as Exhibits
“G” ;

9. She has attached herewith and made integral part hereof her specimen
signatures and the true impressions of her dry seal as exhibits “H”, “H-1”, H-
2” and “I” , “I-1” “I-2”;

10. She has acquired , as an Overseas Workers Welfare Administration (OWWA)

employee, her Authority to Practice from the Office of OWWA
Administrator, a copy of which is hereto attached as Exhibit “J”;

11. She possesses all the qualification and none of the disqualification as a Notary
Public;

12. She has not applied for commission before any other jurisdiction;

13. She undertakes to inform the Executive Judge within ten (10) days in case of
any changes in the given address and to submit her monthly report religiously;

14. She now desires to be granted Notarial Commission for the province of
Cagayan and in the City of Tuguegarao.

PRAYER

WHEREFORE, in view of the foregoing premises, it is respectfully prayed of this

Honorable Court, after due notice, publication and hearing to grant this Petition for
Appointment of herein Petitioner for Notarial Comission for the province of
Cagayan and in the City of Tuguegarao.

Other just and equitable reliefs are likewise prayed for.
