AFFIDAVIT OF SELF-ADJUDICATION

I CIPRIANO BATUNGBAKAL, Filipino, of legal age, single, and a residence of Brgy., Malabanban
Sur, Candelaria Quezon, Philippines after having been duly sworn to in accordance with law, do hereby
depose and state, THAT:

1. I am the sole heir of the late Spouses GENEROSO BATUNGBAKAL and ELADIA BATUNGBAKAL
being his only biological son;

2. The late spouses passed away on 1 November 2016 and died without any last will and
testament;

3. At the time of death my parents had no debts, liabilities or obligations to any persons,
agency or institution;

4. The properties left by my parents are;

a.) House and Located at Betterview Subd., Lucena City, with TCT No. 2468, with an
area of 500 sq.m., valued at Php 6,500,000.00;
b.) Residential Lot with TCT No. 11223 located at Poblacion, Tayabas City, with an area
of 310 sq.m, valued at Php 1,000,000.00;
c.) Agricultural Land under TCT No. 34455, located at Brgy. Binaha, Pagbilao, Quezon,
with an area of 3 hectares, planted with palay and coconut, valued at Php
3,000,000.00;
d.) 4-door Apartment compound for rent located at Candelaria, Quezon with TCT No.
67786, valued at Php 5,000,000.00;
e.) Time deposit with China United Bank under CTD No. 678910 amounting to Php
5,000,000.00 plus interest;
f.) Joint Savings account with Banco Filipino under SA No. 98765 amounting to Php
2,350,000.00 and
g.) Share Capital of Generoso and Eladia with United People’s Multi-Purpose
Cooperative amounting to Php 250,000.00 and Php 300,000.00 respectively.

5. As sole heir of the herein decedent, I hereby adjudicate the above mentioned property
solely for and in my name as well as any and all liabilities arising from the said properties;

6. I hereby execute this affidavit for the purpose of processing the transfer of the said
properties in my name, the settlement of the estate of the late Spouses GENEROSO
BATUNGBAKAL and ELADIA BATUNGBAKAL as well as for the release of any other claim or
benefit in relation to their death before any government/private office and banking
institution and for any other legal purpose this may best serve.
