JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, FELICIDAD H. COLLANTE, of legal age, Filipino citizen and a
resident of Barangay Palimbo Caarosipan, Camiling, Tarlac, Philippines, and
ELENA S. MARTIN, of legal age, Filipino citizen and a resident of Barangay
Bobon 1st, Camiling, Tarlac, Philippines, under oath, do hereby depose and state
THAT:

1. We both know personally RICARDO LORENZO MANGROBANG, JR.

(Ricardo, for brevity), who was born on November 21, 1986 to Spouses
Ricardo A. Mangrobang, Sr. and Simeniana Lorenzo in Camiling,
Tarlac, Philippines;

2. Ricardo had been using ALEGRIA as his Middle Name since he

entered school. Thus, all his pertinent documents including his
Certificate of Marriage and Social Security System (SSS) record bear
such middle name;

3. Recently, when Ricardo secured a copy of his Certificate of Live Birth

from the Philippine Statistics Authority (PSA), Manila, he discovered
that his Middle Name appearing therein is LORENZO;

4. While, in the Certificate of Marriage of Ricardo the recorded name of his

mother is MAXIMINIANA L. ALEGRIA, instead of SIMENIANA
LORENZO. Hence, the discrepancies;

5. We are not related to Ricardo by affinity or consanguinity, except that of

being his barangay mates and acquaintances;

6. We are executing this affidavit to attest to the fact that the names
RICARDO LORENZO MANGROBANG, JR., appearing in the said
Certificate of Live Birth, RICARDO A. MANGROBANG, JR. or
RICARDO ALEGRIA MANGROBANG, JR., appearing in his Certificate
of Marriage and SSS record, refer to one and the same person, and
that is our barangay mate and acquaintance, Ricardo Lorenzo
Mangrobang, Jr.;

7. Moreover, we are executing this Affidavit to attest to the fact that the
names SIMENIANA LORENZO, as the mother of Ricardo Lorenzo
Mangrobang, Jr., appearing in the said Certificate of Live Birth, and
MAXIMINIANA L. ALEGRIA, as the mother of Ricardo A. Mangrobang,
Jr. appearing in the said Certificate of Marriage, refers to one and the
same persons, and that is the mother of Ricardo Lorenzo Mangrobang,
Jr.;
 8. Finally, we are executing this Affidavit to attest to the truth of the
foregoing statements and for all legal intents and purposes it may
serve.
