AFFIDAVIT

I, PRECILLA GAMPON APOSTOL, of legal age, Filipino citizen,

widow and a resident of Kipping Village, Barangay Cacamilingan Norte,
Camiling, Tarlac, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. I got married to Porferio F. Apostol on June 14, 1976 in San

Clemente, Tarlac;

2. When I secured a copy of my Marriage Contract from the

National Statistics Office (now Philippine Statistics Authority), It
was discovered that there was no entry as to the date of my
marriage;

3. I am executing this affidavit to request the local civil registrar

and the PSA to supply the missing entry in my Marriage
Contract particularly the Date of my Marriage which was
celebrated on JUNE 14, 1976;

4. Finally, I am executing this affidavit to attest to the truth of the

foregoing statements and for whatever legal intents and
purposes this may serve;

IN WITNESS WHEREOF, We have hereunto affixed our

signatures this 10th day of January, 2018 at Camiling, Tarlac, Philippines.
