JOINT AFFIDAVIT OF LEGITIMATION

We, DIOSDADO U. BEROLA, SR. and CONRADA B. BEROLA, both Filipinos, of

legal age, married and presently residing at Brgy. Fatima, Villaba, Leyte, Philippines, after
having been duly sworn to in accordance with law, do hereby depose and state:

1. That we have been legally married to each other on March 2, 2017 at a wedding
ceremony solemnized by Municipal Mayor Juliet Larrazabal. A copy of our marriage certificate
is attached hereto and made an integral part of this affidavit;

2. That prior to our marriage and outside of wedlock, a child, named GELYN
BOHOLANO BEROLA, was conceived and born on October 4, 1990 at Mahanglad, Villaba,
Leyte, Philippines;

3. That at the time of the conception of said child, we were not disqualified by any
impediment to marry each other, and, therefore, by virtue of our subsequent marriage, the said
child is now legitimated by operation of law, particularly Article 177 of the Family Code;

4. That we execute this affidavit to declare the truth of the foregoing facts and for
whatever legal purpose/s that this may serve.
