1.

Confined Space Entry


Entry into confined space will only proceed when:
a. all other options have been ruled out
b. permit is issued with authorisation by a responsible person(s)
c. permit is communicated to all affected personnel and posted, as required
d. all persons involved are competent to do the work
e. all sources of energy affecting the space have been isolated
f. testing of atmospheres is conducted, verified and repeated as often as defined by the risk assessment
g. stand-by person is stationed
h. unauthorised entry is prevented
2. Management of Change
Work arising from temporary and permanent changes to organisation, personnel, systems, process,
procedures, equipment, products, materials or substances, and laws and regulations will proceed only when
a Management of Change process is completed, where applicable, to include:
a risk assessment conducted by all impacted by the change
development of a work plan that clearly specifies the timescale for the change and any control
measures to be implemented regarding:
- equipment, facilities and process
- operations,maintenance,inspection procedures
- training, personnel and communication documentation
authorisation of the work plan by the responsible person(s) through completion
3. Ground Disturbance
Work that involves a man-made cut, cavity, trench or depression in the earth's surface formed by earth
removal will only proceed when:
a hazard assessment of the work site is completed by the competent person(s)
all underground hazards, i.e. pipelines, electric cables, etc., have been identified, located and if
necessary, isolated
Where persons are to enter an excavation:
a confined space entry permit must be issued if the entry meets the confined space definition
ground movement must be controlled and collapse is prevented by systematically shoring, sloping,
benching, etc. as appropriate
ground and environmental conditions must be continuously monitored for change
4. Lifting Operation
Lifts utilising cranes, hoists, or other mechanical lifting devices will start only when:
an assessment of the lift has been completed and the lift method and equipment has been determined
by a competent person
operators of powered, lifting devices are trained and certified for that equipment
rigging of the load is carried out by a competent person(s)
lifting devices and equipment has been certified for use within the last 12 months
load does not exceed dynamic and static capacities of the lifting equipment
any safety devices installed on lifting equipment are operational
all lifting devices and equipment have been visually examined before each lift by a competent
person(s)
5. Energy Isolation
Any isolation of energy systems mechanical, electrical, process, hydraulic and others will only proceed
when:
the method of isolation and discharge of stored energy are agreed and executed by a competent
person(s)
any stored energy is discharged a system of locks and tags is utilised at isolation points
a test is conducted to ensure the isolation is effective
isolation effectiveness is periodically monitored
6. Permit to Work
Before conducting work that involves confined space entry, work on energy systems, ground disturbance
in locations where buried hazards may exist, or hot work in potentially explosive environments, a permit
must be obtained that:
defines scope of work
identifies hazards and assesses risk
establishes control measures to eliminate or mitigate hazards
links the work to other associated work permits or simultaneous operations
is authorised by the responsible person(s)
communicates above information to all involved in the work
ensures adequate control over the return to normal operations
7. Driving Safety
All categories of vehicle, including self-propelled mobile plant, will be operated only when:
vehicle is fit for purpose, inspected and confirmed to be in safe working order
passenger number does not exceed manufacturers design specification
loads are secure and do not exceed manufacturers design specifications or legal limits
seat belts are installed and worn by all occupants
safety helmets are worn by riders and passengers of motorcycles, bicycles, quads, snow-mobiles
and similar types of vehicle
Drivers are authorised and:
they are trained, certified and medically fit to operate the class of vehicle
they are not under the influence of alcohol or drugs, and are not suffering from fatigue
they do not use hand-held cell phones and radios while driving
8. Working at Heights
Working at heights of two metres or higher above the ground will only proceed when:
a fixed platform is used with hand rails, verified by a competent person or
fall arrest equipment is used that has:

1. a proper anchor - preferably overhead


2. full body harness using double latch self locking snap hooks at each connection
3. synthetic fibre lanyards
4. shock absorber

fall arrest equipment will limit free fall to two metres or less
a visual inspection of the fall arrest equipment and system is completed and any equipment that is
damaged or has been activated is taken out of service
person(s) are competent to perform the work
1. High-Risk Situation
Do not start up or shut down equipment or installations without using the appropriate, written operating
procedure.

2. TRAFFIC: Machinery/Vehicles/Cyclists/Pedestrians
Do not exceed the speed limit.

3. Body Mechanics and Tools


Do not carry out work if you do not have the right tools for the job and the environment.

4. PPE
Do not access installations or perform work without wearing general or task-specific PPE.

5. Work Permit
Do not perform work without a valid work permit.

6. Lifting
Do not walk or stand under a load while lifting is taking place.

7. Working on Powered S ystem


Do not perform work without checking that the power and product supply has been rendered inoperative.

8. Confined Spaces
Do not enter a confined space until isolation has been verified and the atmosphere checked.

9. Work at Height
Do not work at height without a safety harness when there is no collective protective equipment.

Full body harnesses and safety lines when working aloft.
2. Personal Protective Equipment
Use personal protective equipment for the task at hand.
3. Safe Work Method Statement
Adhere to Safe Work Method Statements (SWMS) at all times.
4. Electrical Safety Guards
Use electrical safe guards and comply with local regulations when working with or near electricity.
5. Worksite, Pit & Manhole Guarding
Guard all pits, manholes and work sites that may present hazards to others.
6. Gas Detection
Use a calibrated (4 gas sensing) gas detector before accessing manholes or
hot works in pits.
7. Vehicle Safety Belts and Road Rules
Wear seat belts when driving and obey all other road rules.
8. Reporting of Incident and Hazards
Report all injuries/incidents and near misses immediately to your Manager.
9. Training and Accreditation
Maintain required mandatory training and apply the current safe systems of work.
10. Confined Space
Do not enter a Confined Space unless certified.

Melakukan analisa bahaya dan bicarakan dalam toolbox talk sebelum melakukan pekerjaan.
2. Kaitkan saat bekerja di ketinggian
Saat bekerja 2 meter diatas permukaan tanah kaitkan anchor penahan apabila tidak ada pagar
pengaman/penghalang. Selalu gunakan pengaman atau double lanyard harness
3. Alat perlindungan jatuh
Alat pelindung jatuh tidak tetap harus terbuat dari bahan kuat dan dapat menahan beban kuat yang dapat
menahan beban 100kg pada ketinggian 600 mm/0,6 meter.
4. Peralatan Listrik
Mengacu pada PTW. Sebelum bekerja pada instalasi listrik pastikan terisolasi, teruji dan di grounding.
Saklar kabel extension dan seluruh peralatan ditata dan dikontrol.
5. Scaffolding dan Tangga
Peralatan buatan sendiri harus bersertifikasi dan diberi label aman oleh manajer konstruksi. Scaffolding
dan tangga harus memenuhi standar internasional.
6. Housekeeping, Barang Berbahaya & Hygiene
Atur tempat kerja/istirahat rapi dan bersih. Toilet terjaga kebersihan & kerapiannya, diperiksa setiap
hari.
7. Ruang Terbatas
Pekerjaan ruang terbatas sesuai dengan standar Internasional
8. Kencangkan sabuk Keselamatan
Pakai sabuk keselamatan pada saat berkendara ditempat kerja. Dilarang bekerja dalam keadaan mabuk
minuman keras dan pengaruh obat terlarang.
9. PPE
Gunakan PPE sebagaimana dijelaskan dalam Safety Induction dan Risk Assessments.
