I, MELODINA ARCIAGA y JOSER, of legal age, married, and a

resident of Santa Maria Drive Bagong Bayan Taguig City, after having
been duly sworn to in accordance with law, hereby depose and state
that:

1. I am the legitimate child of CONCHITA A. ARINO and

VIRGILIO JOSER born on April 6, 1969 at Saint Bernard ,
Southern Leyte;

2. My maiden name was MELODINA A. JOSER.

3. When I was about to secure a copy of my Birth

Certificate at NSO Quezon City, I discovered that I was not
registered at all.

4. When I asked my siblings why my birth certificate was

not registered, I found out that I was not born at a hospital
and was born only through mang-hihilot. And the mang-
hihilot at our barrio was not able to register my date of birth
at NSO.

5. This is the reason why I was not registered at the NSO.

6. I am executing the foregoing Affidavit to attest to the

truthfulness of the foregoing facts and for whatever legal
purpose it may serve.
