Assessment:

- 45-year-old female, diagnosed with high cholesterol 6 months ago

- Height: 65 inches, Weight: 135 pounds, BMI 22.5 (healthy)

- Lab tests report high levels of cholesterol (233.0 mg), HDL and LDL levels are both
elevated as well

- Patient is currently following a high fiber diet

- 24-hour recall reveals intake of 1120 kilocalories, 40 grams fat, 158 grams
carbohydrate, and 55 grams of protein

Diagnosis:
- Inadequate fiber intake related to food and nutrition knowledge deficit
concerning desirable quantities of fiber as evidenced by the patients dietary
recall, low levels of fiber recorded and answers recorded during nutrition
focused interview.

- Inadequate fluid intake related to food and nutrition knowledge deficit


concerning appropriate fluid intake as evidenced by the patients dietary recall
and low amount of fluid intake.

- Limited adherence to nutrition -related recommendations related to perception


that lack resources (time and financial) as evidenced by answers recorded
during nutrition focused interview.

Intervention:
1. Nutrition education to educate patient of proper fiber intake and beneficial
high fiber foods

2. Nutrition intervention of food/nutrient delivery to increase fluid


consumption

3. Nutrition counseling to as assist patient in goal setting to properly use time


and money to build a diet that will assist in lowering cholesterol.
Monitoring/Evaluation:
1. Patient states she has been attempting to maintain a high fiber diet, but
it is not consistent throughout the entire week. She has increased her
fluid intake, but it is still below the DRI for fluids.

2. Patient will continue to intake high fiber foods as well as increase


intake of fluids.

3. Patient will set goals to dedicate more time and money to build a diet
that will assist in lowering cholesterol and will use a self-monitoring
log to track fluid and fiber intake.

4. Process will be evaluated at next visit with RDN
