 Reminder to the Borrowers
1. List all the materials needed (Complete with description, quantity)
2. Check all borrowed item first before leaving the counter. Any complaint about
the item issued will not be entertained once you have left the counter.
3. All borrowers slip should be signed by the Laboratory Custodian.
4. No item, tool, equipment can be taken out of the laboratory premises
without proper authorization from the Laboratory Custodian.
5. Borrowing and returning will be entertained only on the scheduled time.
6. All damage and/or loss equipment should be promptly reported.
Damages and/or losses will be charged to the individual responsible.
However, if the experiment is done by group, charges will be equally divided
among the group members unless special arrangements are made.
