COSMOS is one of the biggest applications in UHG, COSMOS stands for

Comprehensive Online Software for Management and Operational Support.

COSMOS refers to the administrative system software tool that is used to generate
premium bills, adjudicate and pay claims, generate ID cards, maintain eligibility and
provider demographics and contracts, fee schedules, hospital contracts, member
benefits, authorization/referral records, capitation data.

COSMOS is an END to End process. It deals from the point where claim enters into
the system to check write. The major processes involved are Claim Entry,
Adjudication, Check Write, Calc Process.

In COSMOS Development we get various business changes requirement,

enhancements to existing functionalities and introduction of new functionalities in
terms of SPRF’s.

2. COSMOS Field Expansion

Description:

The COSMOS application has been in existence for several years and has been
chosen as one of the strategic claims processing engine for the government book of
business .It has a tremendous growth opportunities in coming years, especially in
expectations of getting TRICARE contracts. But it has some technical limitations
which prevent or hamper its ability to grow further among several business
segments. One of such limitations was its platform and that has been taken care of
porting it from Unisys to IBM.

Another major limitation is the current data model that limits the possibility of
scaling this application for a wider range of business applications. To address this
limitation certain key fields have been identified for expansion and will be
implemented through this project. As these field expansions impact a huge number
of Cosmos programs as well as large number of interfacing applications, it has been
decided to put the programs into several iterations/groups for faster
development/testing and better controlling. The total projects have been divided into
five iterations; each iteration has several groups of programs. For each
iteration/group there will be one separate technical spec so that once it is completed
& reviewed, coding/testing can be started without any delay.

o Iteration 1 - This consists of the Tables and Conversion Screens. The

COSMOS application is predominantly soft coded and all these rules
are available on the Table Online systems. All those programs which
are related with tables and screens are in this iterations

o Iteration 2 - Once the configurations are set up in the tables COSMOS

application will be ready for receiving feed from various systems. This
Iteration consists of data loading applications such as NDB, CPDB and
Facets for Provider Load; similar applications for the DRG numbers to
be loaded. Once the Provider & Contract information is available, we
then have the Claims systems operational. This will include the 837
transactions as well as the online systems on the CPW application.

o Iteration 3 - Once the Claims are entered and loaded with increased
field size the Calc and Check write process can be ready for
processing. All the CALC and Check writing programs are in this
iteration.

o Iteration-4 - All the report related programs are in this iteration.

o Iteration 5 - All the external interface and extracts programs are in

this iteration.
Each iteration has several groups of programs and each group of programs will be
reflected in separate tech spec. The design and development of each group of
program of a particular iteration will continue simultaneously.

Description:

Consumer Database Program is aimed at creation of a single repository of all

consumer data that will be used by the enterprise as the database of record for all
eligibility, consumer activities and servicing. This repository will help United Health
Group to develop the new and leading capabilities that will transform the Traditional
Employer Based Model to a consumer Centric Model. These capabilities will result
into development of new value based products and services targeted to the
individual consumer; improve consumer interaction and low operating costs.

The entire CDB program is split into multiple phases, which delivers the major
functionality over several years. Again each phase is split into manageable releases,
which illustrates a stepping stone approach in building out the database and
subsequent servicing capabilities as well. The purpose of CDB is to develop a next
generation consumer data repository containing consumer and eligibility information
at an individual level for use by all United Health Group businesses.

This Database is intended to contain attributes that allows UHG to manage multiple
and concurrent relationships with each consumer and will be maintained and will also
be available for use by all of United Health Group. The CDB Application has DB2 as
the “Back End Database”. CDB accepts an Input File format from different “Source
Systems” within UHG and validates the data provided. The Data from Different
Source Systems is loaded into CDB.
