NURS 2021H Clinical Course Final Evaluation

Progress
Course Objective Evidence/Indicators Satisfactory Unsatisfactory
Recognize and begin to navigate the - Exposure to young first-time parents (for example, I had a
patient who was the same age as me)
complexities of family nursing. - Experience with a baby diagnosed with NAS who was
apprehended from her parents by CAS they were not
allowed to visit without a CAS representative and once
released from the hospital, the baby will go to a foster home
- gained experience with a family whose first language is not
English (ie. managed to interact with a patient with a
language barrier by using simple sentences and speaking
clarly)
Integrate knowledge from previous - bring in knowledge about common drugs given and their
courses to support diverse side effects from NURS 2550 (ie. Tylenol having no anti-
populations. inflammatory properties)
- sharing with parents the information learned in child
development (PSYC 2500) (ie. babies are born with 20/200
vision and prefer the sound of their mothers voice)
- incorporated skills learned in 2001 about relational inquiry,
cultural safety and being reflexive in practice
Critically appraise relational inquiry - connective with patients on a holistic level to understand all
processes and begin to develop aspects of their culture and needs (ie. asking what is
meaningful relationships with health important to them)
care providers and family members. - seeing the patient as a human being and not just their
diagnosis (ie. a patient being a grandmother of 4 not just the
patient on whom I watched a catheter insertion)
- I continue to greet each patient with a non-judgmental
attitude in order to create a therapeutic relationship (ie. not
judging a mother for using drugs while pregnant even
though I know it is bad)

Collaboratively formulate a plan of - When introducing myself to pt.s I make sure to explain
care based on knowledge of family what I have to do on my shift and when I will be doing
nursing, related theories and scholarly them (ie. bilirubin at 11am and then a bath closer to 3pm)
literature.
- Continue to perform and chart assessments for the head
circumference and intrathecal protocol (q4h)
- Adjusting the current plan of care to work around visitors,
feedings and rest periods (ie. was supposed to do a bath but
then the babys sugars dropped low and the bath had to be
rescheduled).
Develop, implement and evaluate the - carried out discharge teaching such as information about
effectiveness of health-promoting, shaken baby syndrome and keeping the baby safe when
evidence-based practice, reflecting they are sleeping when discharging a patient
principles of family nursing as - informed patient that co-sleeping is not recommended as it
can be dangerous as the baby could fall or be rolled on etc.
relational practice. - talked and educated patients on what kind of pain to expect
when recovering and other alternative techniques to try to
relieve pain (ie. suggesting ice for the perennial area as well
as trying to get up and move around)
Demonstrate increasing competence - I got the chance this semester to remove a catheter from a
and confidence in the application of patient who had a C-section
psychomotor skills in practice - Also had the chance to learn from the nurse how to hook
settings. and unhook an IV for a patient who repeatedly needed to go
to the washroom due to the use of a laxative
- In the NICU, I got the chance to bottle feed and burp, take
vitals and change the O2 sat. monitors on the babies. I also
prepared a dextrose solution into syringes for the premature
babies IVs.
- I also got to see a CT scan that found a kidney stone as well
as an ultrasound on a leg looking for Deep Vein
Thrombosis
- performed a baby weight after removing a hugs tag before
discharging a patient
Demonstrate accountability and - I feel as if the nurses can count on me to perform
professionalism that is consistent assessments or other tasks for them (ie. can you help ___ to
with a nurse entering a self-regulating the toilet or can you take vitals on ____)
profession. - Ability to react professionally in a situation that may be
upsetting (ie. the baby with NAS)
- expected to keep in terms with confidentiality (ie. not using
patient identifiers when talking in the cafeteria at lunch)
Select appropriate community - got to experience Heidi, the lactation consultant work with
support services for families needing a patient
referral to enhance coping with - interacted with the ultrasound technician to learn about the
diverse transitional experiences. process of looking for a DVT
- got to learn and experience the protocol for CAS having
custody over a child

GOALS:
I would like to become more confident when interacting with patients and balancing teaching while I
perform their assessments I believe I can achieve this with lots of practice doing assessments on
multiple patients.
o I believe I have achieved this to some extent as I feel confident performing assessments. I can continue to
develop confidence with patient education by practicing teaching all patients I encounter
I would also like to be able to manage my time more efficiently by creating a more detailed plan of care at
the beginning of the shift and asking the co-assigned nurse if they have specific other things that they
need me to do.
o I believe I have achieved this because I have recently been creating plans of care to ensure I have
performed all the assessments that are needed to be done. As further learning, I can practice checking in
on my patients in a timely manner and stay more organized with a to do list.
