APPLICATION FOR PROBATION

COMES NOW, the Accused, by her undersigned counsel, unto this

Honorable Court, hereby submits this Application for Probation, and further
alleges:

a. That the accused is of legal age, with address at

________________________;

b. That the accused pleaded guilty to a lesser offense as Bettor under

Sec. 2(a) in relation to Sec. 3(e) in lieu of the offense of Collector under Sec.
2(g) in relation to Sec. 3(d) of Republic Act No. 9287 otherwise known as
An act increasing the Penalty for Illegal Numbers Game which she was
charged in the Information;

c. That the accused was convicted of the crime of Bettor under Sec.
2(a) in relation to Sec. 3(e) and sentenced to suffer the straight penalty of
imprisonment of Ninety (90) days;

d. That the accused had not been previously convicted of any crime
and the present crime of which he was convicted is not against the security
of the state;

e. That the accused had not previously availed of the benefits of

Probation Law (Pres. Decree No. 968), as amended;
 f. That the accused is willing to comply with all the requirements,
conditions and terms of the said law.

WHEREFORE, it is hereby prayed that the foregoing Application

be granted and the present case be referred to the City Probation Office for
the Post-Sentence Investigation and Recommendation.
