=eGET A CLAIM Form: the policy holder or beneficiary needs to get a
claim form by phoning Hollard at 0860 737 777. If they have a problem
getting through they can go to any AVBOB or DOVES funeral parlor that
will assist in the claims process. Alternatively they go to any of our PEP
stores who can print them a form from the back office.
GET THE DOCUMENTATION READY:
* Acertified copy of the ID document of the deceased_

A certified copy of the death certificate.
* The BI-1663 certificate.
FAX INFORMATION: The policy holder or beneficiary needs to fax the
claim forms to 011 836 6576. Thereafter the documentation needs to be
posted to:

PAY OUT: An Official claim has been made by the policy holder or
beneficiary if
* An insured person dies from natural causes and has paid premiums for
least 12 consecutive months.
An insured person commits suicide and has paid premiums for at least
consecutive months.
Details of insured person are correct.
