AFFIDAVIT OF BUSINESS CLOSURE

I, RITZIE P. ABES, of legal age, Filipino, married and with residence

and postal address at 105 Maglabe Drive, Brgy. Guinhawa South, Tagaytay
City, after having been duly sworn in accordance with law, hereby depose and
say:

1. That I am the legitimate owner of a duly registered business LAI-TANG

TRADING with former business address at Brgy. Galicia 2, Mendez,
Cavite;
2. That such business has been properly registered with the government
agencies as mandated by law;
3. That such business has not been operational due to its closure since
October 13,2013;
4. That such business has been closed because it was not anymore
profitable, thus its continuation in the business would entail significant
losses; and
5. That I am executing this Affidavit to attest the truth of the foregoing
and for whatever legal purpose this may serve and specifically for
compliance purposes as required by the Bureau of Internal Revenue and
other government agencies for the proper cessation of this business.
