Chapter: Problem:
The associates at New Century Health Clinic approved your recommendations for a new computer system. Your
next step is to develop a design for the new system, including output and user interface issues.
Background
To complete the output and user interface design for the new information system at New Century, you should
review the DFDs and object-oriented diagrams you prepared previously, and the rest of the documentation from
the systems analysis phase. Perform the following tasks.
Assignments
1. Dr. Jones has asked you to create a monthly Claim Status Summary report. He wants you to include the
insurance company number, the patient number and name, the procedure date, the procedure code and
description, the fee, the date the claim was filed, the amount of the claim, the amount of reimbursement, and the
amount remaining unpaid. He wants you to group the data by insurance company number, with subtotals by
company and grand totals for each numeric field. When you design the report, make sure to include a mock-up
report and a report analysis form.
2. Design the daily appointment list and a monthly statement to make it readable and visually attractive. Include
a mock-up report and a report analysis form for each report.
3. Determine the data required for a new patient. Design an input source document that will be used to capture
the data and a data entry screen to input the information.
4. What data validation checks would the clinic need for the new patient data entry screen? Write a brief memo
with your recommendations.

After providing with all necessary details only then he can be authorized to claim any amount to which he is
liable. Group the data by insurance number with subtotals and grand total for each numeric field. By clubbing
these two data together he can save his time and effort and need not refer back for each and every time.
The form layout also includes generation of mock-up reports of an insurer which gives a clear view of person

summary reports helps in gathering the overall information about the insurance and its claim.

A user should be able to place the insertion point only in places where data is entered. Allow users to enter the
data in text box. Typically, white boxes show the location and length of each field. An end keystroke required
for all field. Pressing the enter key or the tab key should signify the end of a field entry.
Users are not required to type zeros before starting a number, may be in integer or decimal format like 089 or
89.00. Provide a way to leave the data entry screen at any time without entering the current record. Users must
also have an option to enter valid entry of data by displaying a message such as, Add this record? (Y/N). A
positive response (Y) adds the record, else it truncates the record.
Users can add or change or delete or view records and able to modify records.
1. Existence check: An existence check is used for those data items which are mandatory. For example, if a
student has a roll number and should allow the student only when he provides roll number else the record will
not be saved.
2. Data check: A data check is s validation that ensures that the data which is to be entered follows a proper
format. For example, data entered in Social Security number might contain both numeric and alphabetical fields.
Hence, the user is required to enter the data both in alphabets as well as numbers, failing to enter any one of
them will lead mislead of data and then data type check validation is performed.
3. Range check: A range check validation is used only when the given data falls in specified range given by
user. Consider an example, employee daily hours work,must be in a range of 0 to 24. Hence it checks for value
entered by user. If the user enters value 25 then it displays error and performs limit check.
4. Reasonableness check: A reasonableness check identifies values that have reasonable value, but need not be
necessarily wrong.
5. Validity check: A validity check is a validation method used for validating certain values. For example, in a
student registration system there are 20 data classes, and if any item of the input data does not satisfy then it
may leads to failure of validity check. Ensure student number on a record matches. The value in one record must
match with another value in another record to check referential integrity. Another validity check might also be
verifying the new student record which is not present in master file.
