Guyabano St. Northridge Subd. Brgy. Sta. Monica, Novaliches, Quezon City, after having been
sworn to in accordance with law hereby depose and state that:

That I personally know SAMUEL P. SUBIA, resident of Block 8 Lot 12 Phase 10

Guyabano St. Northridge Subd. Brgy. Sta. Monica, Novaliches, Quezon City, being his friend.

1. That On December 12, 2015, Samuel P. Subia and Faye Ann R. Dela Cruz celebrated
their marriage at Branch 78 Regional Trial Court, Quezon City.

2. That I knew these facts because I was personally present and signed as witness to the
said marriage.

3. I herein attest to the truthfulness of the foregoing facts.
