I, ATTY. PIOLO RAYDA, of legal age, married, Filipino, and a resident of 123 San Marcelino
Street, Paco, Manila, under oath, hereby depose and state that:
1. On January 20, 2010, at or around 9:00 p.m., I, together with several friends and
officemates, went to Booze Stop Bar and Grill located at Kamagong Street, Makati City for
dinner and a couple of drinks and went home at or around 1:00 a.m. of January 21, 2005;
2. On January 21, 2010, I arrived at my residence at or around 1:00 a.m. and went straight up
to the bedroom and fell asleep shortly thereafter;
3. I awoke from my sleep because of a sudden pain in my genital area and I was bleeding
profusely; and that I realized that my penis had been cut off by my wife, ZORAH RAYDA,
whom I saw was holding a bloodied knife;
4. About thirty (30) minutes later, I was rushed to the UP-PGH Hospital at Taft Avenue, Manila
because of my condition;
5. I incurred expenses by reason of my treatment, and the receipts thereof are herein
attached;
6. I am executing this affidavit-complaint in support of the filing of an appropriate criminal
action against my wife, ZORAH RAYDA, in a court of law and for other legal intents and
purposes.
