Copyright Transfer Agreement

The Taiwanese Dermatological Association will be pleased to publish your article entitled:

in Dermatologica Sinica (the Journal) if the Work is accepted for publication. The undersigned authors transfer all copy-
right ownership in and relating to the Work, in all forms and media, to the Taiwanese Dermatological Association in the
event that the Work is published in the Journal. However, this agreement will be null and void if the Work is not published
in the Journal.

The undersigned authors warrant that the Work is original, does not infringe on any copyright or other proprietary right of
any third party, is not under consideration by another journal, and has not been previously published.

This statement is signed by all the authors listed in the manuscript to indicate agreement that the above information is
true and correct (a photocopy of this form may be used if there are more authors):
