Patient : My name is Mahkda Anjani Putri, you can call me Mahkda
Nurse 1 : Oh, hello Miss Mahkda. My name is Riska Utama you can call me nurse Riska,
and than my partner
Nurse 2 : Hello my name is Ratnasari Hardiyanti, you can call me Ratna
Nurse 1 : What brought you to the hospital? (providing general lead-reason go to the
hostipal)
Patient : Last three weeks, I've done fracture surgery on my right hand due to a single
accident.
Nurse 2 : Could you tell me more about that? Before that can we get more information
about you? (Seeking information). We have some question for you to complete
your identity data.
Patient : Yes, no problem.
Nurse 1 : How old are you? (Open-ended question about personal identity)
Patient : I'm ninety years old
Nurse 1 : Where dou you live? (Open-ended question about personal identity)
Patient : I live on Gembili Raya street, No. 77 A, Surabaya
Nurse 1 : Are you married? (Open-ended question about personal identity)
Patient : No, I'm single
Nurse 1 : What is your occupation? (Open-ended question about personal identity)
Patient : I'm student college
Nurse 1 : How do you feel now? (Open-ended question about present illness)
Patient : After I had surgery fractures. I feel pain in my right hand and was difficult to be
moved.
