STUDENT FEEDBACK FORM

Dear Year 12 Outdoor & Environmental Studies students.
As part of my university placement I am required to undertake a small project and
implement it into the classroom to benefit you, the students. As you are aware you were
given a multi-page handout to help you revise for UNIT 3 SAC 2 which I prepared which I
hope you found beneficial, and now I would really appreciate your feedback.

When completing this feedback form please give me your HONEST opinions about the
revision handout as it will only help me to become a better teacher in the future.
To help you remember, the SAC Revision handouts you were given consisted of the
following 6 parts:
