4.1 Documentary Requirements

4.1.1 ForNewApplications

5. TBRII{ OF ACCITBDITAIION

The accredit*ion shdl be issued for a tenn of one (l) year

renewable unually.

6. ST'PERVI$ION ITEB

Any accredited cargo-handlor operating within a private port

ehall remit to &e Authority a srryenision fee in the armunt equivalent to
lhree percent (3%) oftheir gross receipts, which shall be remitted to the
Auhority immediately after the conrpletion of the loading $rd/or
unloading ofthe cargo to and/or from the vessel.

7. SOLIDARY RE$PON$IDIIJTY

The private port owner or operator and the accreditEd cargo-

hmdliry operator shall have Bre joint and solidary responsibility of
ensrring pronpt payrnent of zupervision Ge at the rate imposed by
the Autbority.

8. PROEIBMONS.VIOLATIONS

8.lProhibition against Assignment and Transfer of

Accreditation. - There shall be no assignment,
subletirrg, mortgage, or hansfer, direct or indirect of
the Accreditation or any portion thereo{ or flly
uodertaking trereurder without the prior written
approval of the Authorig.

S.2Prohibition agaiust operation outside of private porl

area - The accreditation shall not be valid for
cargo-handling services outside of tlre privale port
which endorsed and contracted the accreditee.

8.3 The private port owner or operalor and/or the

accreditee shall not alter, modi$, or arnend or revise
their service conbact without the prior written
permission of the Ar.ilhority.

8.4 No private port owner or operator or cargo-handling

accreditee sball engage in the "cabo systenL"

8.5 CPA officiale and/or their representatives shall have

visitorial powert over privde ports, and privde port
ownere or operatore ehall at all times allow access
to said C?A officials or employees.
9. TENAITY

The Authorig may curcel the accreditalion for my violalion of ttre terms
md conditiong of lhc Accreditotion or provisions of law, goverrunent rulee urd
regulations.

10. RXPBALINGCLAUSB

All ordors, rulcs and regrlations, memorand4 circulars or guidelinee which

inconeistsqt berewift re hereby repealed or modified eccordiogly.

II. ItrTBCTTVITY

This AdminisMive Order shell take efect immediatelv.
