Description of the artifact and how it meets the standard and criteria:

This artifact is a copy of some math exit slips that students have completed at the end
of a math lesson. These exit slips are something that was provided to me by the school
district through our Building School Improvement Leader and are directly aligned with
district curriculum (GO Math!) and state standards. I am able to use the data that I
collect from these exit slips to guide my instruction for the next class period. On these
exit slips, you can see where I have circled and helped students make changes as they
are learning this new concept. I met with these students in a small group setting where
they had a chance to make changes that they needed to and ask questions along the
way.

Reflection on the artifact:

I have found that using exit slips like these ones are very helpful in guiding my
instruction. They help me to know very quickly which students need a reteach session,
which students just need a concept cleared up quickly, and which students are fully
understanding the content. I like to use these exit slips to form groups on the second
day of a lesson and make sure that I am spending a good chunk of time with the
students that need a reteach session.
