& Leiden LLP (BAL) has been retained for the purpose of representing you, the
beneficiary, your spouse and/or other family members (if applicable), and your U.S.
employer sponsor ("the Company"), if your employer is covering any or all of the fees
associated with this application or petition, in preparing and filing one or more
applications or petitions related to an immigration benefit for you through the Company's
sponsorship. If the Company is compensating BAL for this representation, this fact will

conflict of interest arises, you must advise us immediately. If an actual conflict of interest
arises during the course of our joint representation, we will notify both you and the
Company of such conflict and take appropriate action, which would likely include
withdrawal of our representation of you and your family members, if applicable. You
understand and agree that BAL's legal representation of the Company does not include
disclosure to you of confidential Company information, within reasonable lawful limits,
that the Company wishes to keep confidential. You may seek the advice of independent
counsel regarding this disclosure and consent.

THIRD PARTY SERVICE PROVIDERS NOTICE AND CONSENT: Berry

Appleman & Leiden LLP works with non-lawyer third parties who provide services
including credentials evaluations, visa and work permit information and processing,

passport processing, document acquisition and legalization, translation, relocation, and

other services of an administrative nature. In order to provide these services on behalf of
you and your family, it may be necessary to share with these third parties personal and
family information -- including immigration history, financial information, and
employment information -- that you provide to Berry Appleman & Leiden LLP.
NOTICE REGARDING CHANGES IN EMPLOYMENT: In the event that you, the
principal applicant, cease to be employed with your current employer, BAL reserves the
right to withdraw as attorney of record and cease to represent you and all family
members. Should you cease to be employed by your current employer during the I-
485/Adjustment of Status process, under certain circumstances, BAL may continue
representation during the I-485/Adjustment of Status process under the terms of a
separate retainer agreement and payment for any additional work to be performed by
BAL.

DATA AND FILE RETENTION NOTICE: After our services conclude, Berry
Appleman & Leiden LLP will, as a courtesy to you, make its best efforts to retain the
personal data you have provided and a copy of your file for a period of six (6) years. At
the end of said period, we may, at our discretion, destroy such personal data and file
without notice to you. At any point during the retention period, you may request delivery
