1. That I was born on March 9, 1985 in San Francisco, Cebu, to the

SPOUSES IRENIO BALO SEGOVIA and NICANORA BARCENAS
BOHOL;
2. That in my Certificate of Live Birth issued by National Statistics Office and

Certificate of Baptism issued by St. Joseph Parish in San Francisco, Cebu,
my fathers first name was stated correctly as IRENIO;
3. That in my Certificate of No Marriage issued by Philippine Statistics

Authority, issued on March 20, 2017, my fathers first name was
erroneously stated as ERNESTO instead of IRENIO which is his true
and correct name;

forgoing facts and in order to reflect my fathers true and correct name
