AFFIDAVIT OF INCOME

I, ERWIN BAUTISTA, Filipino, of legal age and resident of Blk. 1, Lot

11, Brgy. Sta. Fe, Extension, Dasmarinas, Cavite, after being duly sworn to in
accordance with law, depose and state: That - -

1. That I was required to declare my monthly income;

2. That I am presently employed as a Product Consultant for

Voyager brand of bags and Luggage;

2. As such, I am earning approximately FIFTEEN THOUSAND PESOS

(P15,000.00) a month;

3. Through this Affidavit I hereby declare under oath that I am

earning approximately FIFTEEN THOUSAND PESOS (P15,000.00) which I
derived from my above-said employment;

4. I am executing this Affidavit to attest to the truth and veracity of

the foregoing and for whatever legal purpose the same may serve;

University-Dasmarias;

2. As such, I am earning approximately TWELVE THOUSAND PESOS

(P12,000.00) a month;

3. Through this Affidavit I hereby declare under oath that I am

earning approximately TWELVE THOUSAND PESOS (P12,000.00) which I
derived from my above-said employment;

4. I am executing this Affidavit to attest to the truth and veracity of

the foregoing and for whatever legal purpose the same may serve;
