AFFIDAVIT

I, TERESITA C. RAMOS, Filipino citizen, of legal age, and resident of Blk 45,
Lot 37 Brgy. San Miguel, Dasmarias City, Cavite, after being duly sworn to in
accordance with law, depose and state: That - -

1. That I am the mother of ENRIQUE R. SAMONTE who was born on

September 18, 1969 at Tondo, Manila;

2. Sometime in 1982, he ran away from home due to misunderstanding with his
father and has never comeback ever since.

3. That his signature was required for the Extrajudicial Settlement of Estate and
documents pertaining to application at National Housing Authority.

4. That since then up to present, I have no communication whatsoever from him

neither that I have any idea of his whereabouts;

5. Through this Affidavit, I hereby declared under oath that my son ENRIQUE
R. SAMONTE ran away from home and since then up to present, I have no
communication whatsoever from him neither that I have any idea of his
whereabouts and for the same reason, his appearance and signature cannot
be acquired for the above said documents.

6. I am executing this Affidavit to attest to the truth and veracity of the

foregoing and for whatever legal purpose the same may serve;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 16th day
of August 2017 in Dasmarinas, Cavite.
