COMPLAINT

PLAINTIFF, through the undersigned counsel, and to this Honorable Court, most respectfully
alleges that:
1. Plaintiff, X, is a Filipino, of legal age, single and residing at 23 Swallow Street, Makati City,
whereas defendant, Y, is likewise Filipino, of legal age, single, and residing at 32 Garbler
Street, Makati City, at which address the parties herein may be served with summons and
other court processes;
2. On 30 May 2017, defendant borrowed from plaintiff the amount of P1,000,000.00, which
indebtedness is due and payable on or before 30 August, 2017, with an interest at the rate
of 12% per annum, in accordance with the promissory note executed by the defendant on
the said date. Photocopy of said note is herewith attached and marked as Annex A and
made integral part hereof.
3. Despite the lapse of the aforesaid period, and after repeated demands, the defendant has
failed and refused and still fails and refuses to pay the said indebtedness on due date, with
the corresponding interest thereon to the herein plaintiff.

WHEREFORE, it is most respectfully prayed of this Honorable Court that after due hearing,
judgment be rendered in favor of plaintiff and against defendant in this manner:
1. Ordering the defendant to pay plaintiff the amount of P1,000,000.00. Plus interest
thereon at the rate of 12% per annum; and
2. Ordering the defendant to pay the costs of this suit.

Such other reliefs as may be just and equitable under the circumstances are likewise prayed
for.
