AFFIDAVIT OF CONSENT

I, SISA A. SHIMONO, of legal age, Filipino, married, with residence

and post-office address at B5, Lot5 Pinewood Village, Imus, Cavite, after
having been duly sworn to in accordance with law, hereby depose and state
THAT:

1. I am the legal and biological mother of JACK N. ALVAREZ

who was born on October 18, 1992;

2. That my son JACK N. ALVAREZ intends to contract marriage

on June 4, 2013;

3. That my son JACK N. ALVAREZ being only 20 years of age.

The law particularly the Family Code requires that he secures
a parental consent before he contract marriage;

4. In view thereof, I being his legal and biological mother gives

my consent to said marriage;

5. I executed this affidavit to attest to the truth of the foregoing

facts and for whatever legal purposes this may serve.
