A. WORK STATION

Shared space at the Social Mobilization cubicle, SGOD Office,2 nd floor,


Teachers Hall (Building 3).

B. ACTIVITIES

Attended the orientation and open discussion on Individual Performance


Commitment and Review Form (IPCRF) for the year 2017 given by Madam
Renata G. Rovillos (Education Program Supervisor, LR) on January 6, 2017.
Assisted in the documentation of Division Seminar-Orientation and
Distribution of First Aid Kit to all Disaster Coordinators held at Teachers Hall
on January 11, 2017.
Assisted in the conduct of Continuous Improvement (CI) Coaching held at
Teachers Hall on January 16, 2017.
Attended the first Division Office Mancom for this year at Abong na Alumni,
DCNHS on January 17, 2017.
Joined the DCAA Awarding Ceremony held at Lyceum Northwestern
University Gymnasium on January 18, 2017.
Providing assistance in the operations of the division office.
Encode and manage data pertaining to Accession Record of Division Library
Collection.
Handle clients and visitors.
Operate photocopying machine.
Maintain cleanliness in the workplace.
C. STATUS REPORT ON LIBRARY HUB

The edifice is located at Bonifacio Street in Barangay Poblacion Oeste, Dagupan City
and completed last May, 2016. Supplementary materials from the Central Office have
not yet arrived though a Letter of Request has been sent to the Office of the Secretary
last year to follow-up the library resources to be provided by the Central Office as
stated in the Memorandum of Agreement.

D. COLLECTION DEVELOPMENT OF DIVISION LIBRARY

The Division Library continues to build its collection thru book donations from private
individuals and NGOs. For the month of January, the library acquired one (1) book
from DepEd-Disaster Risk Reduction Management Service DRRMS entitled What
Happens in Disasters! A Compilation of Real Accounts from Survivors of Different
Natural Disasters thru Mrs. Marjorie S. Rodriguez, PDO II, DRMM.

A. ACTIVITIES

Assisted in the conduct of a two (2)-day Validation of Distribution of K to 12


Teaching and Learning Resources by the Bureau of Learning Resources (BLR)
personnel of the Central Office to the Division on February 7-8, 2017.

Assisted in the conduct of National School Building Inventory (NSBI) at


Dagupan City National High School (DCNHS) led by Mr. Ronie G. Bonao,
Public Schools District Supervisor (PSDS) and Engr. Tatum Grace L. Manzano
on February 14, 2017.

Provided Technical Assistance to the School Library of DCNHS through its


School Librarian Ms. Fe De Cadiz for the enhancement and implementation of
school library services on February 14, 2017.

TECHNICAL ASSISTANCE TO SCHOOL LIBRARIAN. Mr. Jan Vincent D. Galvez,


Division Librarian provided Technical Assistance (TA) to the School Librarian for
the improvement of functionality status of School Library.
Served as a support staff during conduct of Division Capacity Building for
Brigada Eskwela/ Adopt-A-School Coordinators/ partnership Focal Persons
held at Teachers Hall on February 18 and 25, 2017.

B. COLLECTION DEVELOPMENT OF DIVISION LIBRARY

The Division Library partnered with Bagong Kulturang Pinoy (BKP), a


registered non-profit organization based in Massachusetts, USA for the provision of
childrens books and other educational materials for the library.
The organization generously donated varied library resources composed of 334
books, 10 serials and 1 non-printed material to the Division Library of Dagupan City.
The partnership became a success and possible through the support of DepEd
Officials led by Mdm. Froserfina J. Bravo, Schools Division Superintendent and Dr.
Lourdes D. Servito, Assistant Schools Division Superintendent.

ON THE SPOT TURNOVER CEREMONY. Dr. Lourdes D. Servito, Asst. Schools


Division Superintendent led the turnover of book donations to the Division
Librarian on February 27, 2017.

A. ACTIVITIES

Prepared Monthly Reports of the Division Library.


Prepared the complete List of Supplementary Materials donated by Bagong
Kulturang Pinoy, Inc. (BKP)
Updated the Inventory Report of the Division Library Collection.
Prepared the IPCRF for the year 2016.
Prepared a Letter of Acknowledgement of book donations and sent it to the
donor (BKP.Inc.) via email signed by Dr. Lourdes D. Servito, on behalf of the
Schools Division Superintendent, Madam Froserfina J. Bravo on March 16,
2017.
Assisted in celebration of Womens Month: Culminating Program held at
Abong na Alumni on March 29, 2017.
ZUMBA-FUN WORKOUT FOR ALL AGES. Teachers, Division Personnel, and
pupils were actively participated the one (1) hour Zumba Session during the
culminating program in celebration of Womens Month.

Monitored the first Simultaneous Earthquake Drill for this year at T. Ayson E.
School of Barangay Calmay, Dagupan City on March 31, 2017.
AMAZING MOTORBOAT RIDE TO ASSESS THE CAPACITY LEVEL OF AN ISLAND SCHOOLS
IN TIME OF DISASTERS. On our way to our respective assigned island school in Dagupan
City to monitor its earthquake preparedness.

during the conduct of the first nationwide simultaneous earthquake drill at

Assisted during NDEP Coordinators Meeting held at Teachers Hall on April 3,
2017.

Attended an orientation on Work and Financial Plan Activities given by Dr.

Assisted in the conduct of 2017 Drug Symposium held at Boys Scouts of the

WE ARE ALL DRUG EDUCATION ADVOCATES, as everyone is re-oriented on the
countrys drug abuse problem, the ill effects of drug abuse and the legal
consequences of being involved in illegal drugs activities during the recently
concluded Drug Symposium held at Boys Scouts of the Philippines, Dagupan City
Council Building on April 22, 2017.

A. ACTIVITIES

Prepared a memorandum on an upcoming Division Seminar-Workshop on


School Library Management and Operations slated on June 30 to July 1,
2017.
Assisted in Consolidating a report on List of schools recipients of science
and math equipment to be submitted in Regional Office as per instruction of
Mrs. Renata G. Rovillos, Education Program Supervisor, LR.
Updated the Accession Record of the Division Library Collection.
Assisted in Distribution of R1AA Uniforms to all City Officials and
Department Heads of City Government of Dagupan as per instruction by Dr.
Marissa O. Perez, Education Program Supervisor of SGOD on May 9, 2017.
Participated in the conduct of Brigada Eskwela 2017 in the Division Office
on May 17, 2017.
Conducted an ocular inspection on a relocation of the temporary Division
Library in the Division Training Center together with Ms. Clarita M.
Tamayo, Supply Officer II as per instruction by Mdm. Froserfina J. Bravo,
Schools Division Superintendent on May 22, 2017.
Assisted in Documentation of Distribution of checks thru DRMM Financial
Assistance to all the recipients- pupils who were victims of typhoon Lawin
on May 25, 2017.
Provided assistance to all pupils, parents and teachers as the school opening
is approaching thru a Help Desks Oplan Balik Eskwela on May 31, 2017.
BATTLE WITH DIRT. Mr. Jan Vincent D. Galvez, Librarian II cleans the
windows leading to the Office of two Chiefs of Schools Division Office-
Dagupan City in observance of this years Brigada Eswela.
HELP DESK-READY TO RESPOND ANY QUERIES & COMPLAINTS. DepEd-
Dagupan City Installs Help Desk to accommodate inquiries and complaints
from all stakeholders as the opening of the classes will start on June 5, 2017.

A. ACTIVITIES

Provided assistance to all pupils, parents and teachers as the school opening
is approaching thru a Help Desks Oplan Balik Eskwela on June 1, 2017.
Followed up the memorandum on Division Seminar-Workshop on School
Library Operations and Management slated on June 30 to July 1, 2017.
Attended Division Office Mancom at Division Training Center on June 2,
2017.
Monitored first day of school opening to make sure that pupils were safe and
sound during the opening classes of Juan L. Siapno Elementary School.

Its school time again for all pupils as Mr. Jan Vincent D.
Galvez, Division Librarian does head count to Grade I pupils
of Juan L. Siapno Elementary School.

Attended a meeting on Monitoring first day of classes presided by Dr.


Lourdes D. Servito, Assist. Schools Division Superintendent together with
two Chiefs-Dr. Venus Maria SM. Estonilo of CID and Mdm. Maria Linda R.
Ventenilla of SGOD held at Teachers Hall at 4pm.
Prepared a Purchase Request and Purchase Order of meals and training
materials together with Ms. Clarita Tamayo, Supply Officer II to be used for
the upcoming first Division Seminar-Workshop for all Teachers-Librarians.

Provided Technical Assistance to ten (10) School Libraries through its


School Librarians/Teachers-Librarians for the maintenance, enhancement
and improvement of school library services.

Served as judge on Division Level Poster making and essay writing contest
held at Teachers Hall on June 28, 2017.
Dagupan City Councilor, Hon. Maybelyn D. Fernandez led
the awarding ceremony.

Monitored the second Simultaneous Earthquake Drill for this year at Juan P.
Guadiz Elementary School on June 29, 2017.

Pupils, teachers, School Head, parents and other


stakeholders participated during the Nationwide
Simultaneous Earthquake Drill.

Prepared and monitored all the needed materials for the conduct of Division
Seminar-Workshop on School Library Operations and Management.

A. MEETING ATTENDED

Attended Kumustahan Meeting presided by Schools Division Superintendent,


Dr. Lorna G. Bugayong at Division Training Center on July 24, 2017.

B. TECHNICAL ASSSISTANCE

Provided Technical Assistance to five (5) School Libraries through its


School Librarians/Teachers-Librarians for the maintenance, enhancement
and improvement of school library services.

HIGHLIGHTS OF IMPROVEMENT OF LEARNING RESOUCE CENTER

Relocation of Malued Elementary School Library in a safer place.

During the conduct of library visitation last November 22, 2016, Division Librarian
recommended to the School Head of Malued Elementary School to immediately relocate
its library in a safer place, since the former location occurred shaking at the 2 nd floor where
the library is previously located. Now, the library is relocated in a safer and bigger space and
currently organizing its collections during the validation of the said relocation on July 25,
2017.

C. BOOK DONATIONS
Division Library has received one (1) big balikbayan box which contained full of
childrens books from Bagong Kulturang Pinoy (BKP, Inc.), a registered non-profit
organization based in Massachusetts in US last July 27, 2017. The Turnover of book
donations to the Division Librarian was led by ASDS, Dr. Lourdes D. Servito. These
supplementary materials will surely benefit to learners as they will learn to love reading
and enhance their comprehension skills.

A. ACTIVITIES

Served as a program manager and one of the resource speakers on the first ever
Division Seminar-Workshop on School Library Operations and Management.

Participants confidently showed their outputs on cataloguing during the second


and last day of training for teacher-librarians on July 1, 2017.

Prepared and submitted monthly report.


Compiled photos of Learning Resource Centers (LRCs) as per instruction by
Assistant Schools Division Superintendent, Dr. Lourdes D. Servito.
Submitted Narrative Report on Division Seminar-Workshop on School Library
Operations and Management.

Attended a meeting to all Curriculum Implementation Division (CID) personnel


presided by Schools Division Superintendent, Dr. Lorna G. Bugayong, CESO
VI together with Assistant Schools Division Superintendent, Dr. Lourdes D.
Servito and CID Chief, Dr. Venus Maria SM. Estonilo on July 10, 2017.
Prepared project proposal and rubrics to be utilized for the upcoming Division
Search for Most Functional School Libraries-Public Elementary level and
Public Secondary level.

Shared story reading to the pupils of Mamalingling Elementary School on July


4, 2017.

Impromptu storytelling has been conducted at the Principal Office as an


alternative way of promoting love for books and reading since the school
library has been temporarily closed.

Grade 6 pupils were both actively and quietly engaged in the storytelling
session held at Mamalingling ES.

Assisted in registration and documentation during Post R1AA/Palaro Meet


Conference held at Teachers Hall on July 5, 2017.
Schools Division Superintendent, Dr. Lorna G. Bugayong, CESO VI
spearheaded the meet conference and wholeheartedly acknowledged the effort
of Dr. Marissa O. Perez as an EPS in charge of School Sports for two (2)
years; she also expressed her full support to the incoming Division Sports
Coordinator, Dr.Jaime Siapno during the Palaro Meet Conference.

Cleaned the Room together with the Utility Worker intended for the Division
Library on July 17, 2017.

Mr. Danny Caguioa of SDO cleaned the floor of the Division Library located at
Division Training Center.

Mr. Danny Caguioa also thoroughly washed and brushed the tiles of lavatory of
the SDO library.
Attended the Orientation on the Establishment, Management and Operations
of the Library Hub on July 19, 2017 held at LRMDC-RO1, San Fernando City,
La Union.

Regional Librarian, Ms. Nids Cabal (standing) presided the orientation and the
first ever meeting of all SDO Librarians of Region 1.

Region 1-SDO Librarians took a group picture with Regional Key Officials led by Assistant
Director, Dr. Bettina Aquino.
Attended Kumustahan Meeting presided by Schools Division Superintendent,
Dr. Lorna G. Bugayong at Division Training Center on July 24, 2017.

Prepared a Letter of Acknowledgement of book donations and sent it to the


donor (BKP. Inc.) via email signed by Schools Division Superintendent, Dr.
Lorna G. Bugayong, CESO VI on July 28, 2017.

Provided Technical Assistance to ten (10) School Libraries through its School
Librarians/Teachers-Librarians for the maintenance, enhancement and
improvement of school library services.

HIGHLIGHTS OF IMPROVEMENT OF LEARNING RESOURCE CENTERS (LRCs)

BOLOSAN ES LIBRARY. Bookshelves are multipurpose since the top


surface is used as alternative baggage counter where the pupils are required to
leave their bags upon entering the library.
BOLOSAN ES LIBRARY. Color coding system is adopted for easy access.

MALUED ES LIBRARY. Relocation of its library in a safer place.

SABANGAN ES LIBRARY. Pupils read books and do research on their
assignments.

T.AYSON ROSARIO ES. Ceiling is being installed to make the library


more conducive in learning and teaching.
BLISS ES LIBRARY. Mother tongue storybooks and other reading
materials were reorganized and showcased

LUCAO 1 ES LIBRARY. Lots of dust in all corners.
VICTORIA Q. ZARATE ES LIBRARY. Dusty books and disorganized
library.

FEDERICO N. CERALDE ES LIBRARY. No existing library due to lack


of classrooms. The old library is now being used as a classroom.
MAMALINGLING ES LIBRARY. The Reading area and Filipino children
literature books were overshadowed by the construction materials such as
cements, lumbers, and used GI sheets.

SECONDARY SCHOOL LIBRARY. The library collections were
disorganized and had no improvements at all, since the last two visits of the
Division Librarian for the last two (2) years.
JUAN P. GUADIZ ES LIBRARY. No existing library due to the on-going
construction of new classrooms.

For the second time around, Division Library Division Library has received one (1) big
balikbayan box which contained full of childrens books from Bagong Kulturang Pinoy
(BKP, Inc.), a registered non-profit organization based in Massachusetts in US on July 27,
2017. The organization generously donated varied library resources composed of 334
books, 10 serials and 1 non-printed material to the Division Library of Dagupan City.
The Turnover of book donations to the Division Librarian was led by ASDS, Dr. Lourdes
D. Servito. These supplementary materials will surely benefit to learners as they will
learn to love reading and enhance their comprehension skills.
Assistant Schools Division Superintendent, Dr. Lourdes D. Servito happily
showcased the book donations composed of a quality reference and fiction
books from the BKP, Inc. on July 27, 2017.

SDO Librarian, Mr. Jan Vincent D. Galvez cheerfully received the book
allocation from the BKP.Inc. during the turn-over of the book donations led by
the Assistant Schools Division Superintendent, Dr. Lourdes D. Servito.
