WAIVER OF RIGHTS

KNOW ALL MEN BY THESE PRESENTS:

That I, BANGITAO JACKSON P of legal age, Filipino citizen, with residence and postal
address at BAGUIO CITY Philippines, do hereby state that:

1. I am the Applicant- in the Application for Approval of SALE AND TRANSFER of a

Certificate of Public Convenience (CPC) or Franchise to operate a TAXI Service, on the
line/route: BAGUIO CITY TO ANY POINT IN CAR with the use of one (1) authorized unit,
described as TOYOTA WAGON with Plate No.AYM 325;

2. That I cannot appear on said hearing date for application of Sale and Transfer due to my
present employment and that I have to focus on some personal obligations.

3. That I am interposing NO opposition or objection to the approval of the said Application

for Approval of SALE AND TRANSFER of a Certificate of Public Convenience (CPC) or
Franchise to operate a TAXI Service by the Board (LTFRB);

4. Finally, I/We do hereby willfully and voluntarily WAIVE my RIGHTS to be present

during the hearing of the said Application at the Land Transportation Franchising and
Regulatory Board (LTFRB), Department of Transportation and Communications
(DOTC), Cordillera Administrative Region (CAR), Baguio City, for all legal intents and
purposes.

In view of the foregoing, it is respectfully requested of the said agency (LTFRB, DOTC-
CAR, Baguio City) to cause the approval of the said Application in favor of the applicant-
PATRICIO EVANGELISTA, for all intents and purposes.

That I personally caused the preparation of this WAIVER of RIGHTS willfully and
