JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

WE, CHRISTIAN P. DOMINGO, of legal age, Filipino citizen,

and a resident of Barangay Cario, Paniqui, Tarlac, and CHARLIE R.
MARULLO, JR. of legal age, Filipino citizen and a resident of Abbellar
St., Dela Pea Subd., Pob. 3, Gerona, Tarlac, Philippines, under oath,
do hereby depose and state THAT:

1. We both personally know MYLYN GARCIA CABANGBANG

(Mylyn, for brevity), born on November 25, 1988, to Spouses
Arsenio Cabangbang and Teresa Garcia in Camiling, Tarlac;

2. Mylyn had been using COBANGBANG as her Middle Name

since she came to the age of reason. As a result, all her
school records, pertinent documents and identification cards
bear such middle name;

3. When Mylyn secured a copy of her Certificate of Live Birth,

she discovered that her Surname and that of her father
appearing in therein is COBANGBANG, hence, the
discrepancy;

4. We are not related to Mylyn either by consanguinity or affinity

except that of being her friends and acquaintances;

5. We are executing this affidavit to attest to the fact that the

names MYLYN GARCIA CABANGBANG, whose fathers name is
Arsenio Cabangbang appearing in the said Certificate of Live
Birth, and MYLYN GARCIA COBANGBANG, as recorded in her
school records, pertinent documents and identification cards,
refer to one and the same person, and, that is our friend and
acquaintance, Mylyn Garcia Cabangbang;

6. Finally, we are executing this Affidavit to attest to the truth of

the foregoing statements and for all legal intents and
purposes it may serve.
 IN WITNESS WHEREOF, we have hereunto affixed our
signatures this 2nd day of February, 2016 at Camiling, Tarlac,
Philippines.
