ESTABLISHMENT IN FRANCE (Short Stay)


Family Member of EU/EEA/Swiss National

All documents must be presented in the order specified below and in two (2) sets: the original and
photocopies

Signed long stay visa application form with photo (35mm x 45mm, white background
Passport with a validity of at least 1 year and photocopy of valid and former visas
Proof of familial ties between the applicant and the EU/EEA/Swiss National
Civil registry documents (marriage certificate and birth certificate)
In case of separation of parents, for minors below 18 years:
o Judgment of full custody
Proof of nationality of the EU/EEA/Swiss family member:
National Identity Card; or
Passport; or
Certificate issued by diplomatic or consular representative in the Philippines; or
Other documents as may be applicable
Proof of residence in France of the EU/EEA/Swiss national
Working contract; or Proof of residence/accommodation in France; or contract of
sale/lease of lodging/residence in France
Letter stating the intention to establish/live indefinitely in France
For descendants above the age of 21 years or parents of an EU/EEA/Swiss national, proof
that they are qualified dependents:
Affidavit executed by the EU/EEA/Swiss national declaring that the visa applicant is
his/her qualified dependent; or
An official document issued by the competent authority of the origin state/country of the
EU/EEA/Swiss family member declaring that the applicant is a qualified dependent.
