The Annual Performance Assessment Report is an important document. Please fill it with care and in manuscript

The objective of this form is to develop the officer and make him/her realize his/her true potential. It is not meant to
be a fault finding process. Reporting Officer should not shy away from reporting shortcomings in performance, attitudes or
overall personality of the officer reported upon.

The form should be filled specifically. No ambiguous and doubtful wordings should be used.

If the Reviewing Authority is satisfied that the Reporting Authority had made the report without due care or attention
or unduly delayed the report, he shall record to that effect in Part IV.

Where any of the Reporting Officers awards a grading unsatisfactory or gives any adverse/remedial remarks/non
recommendation, the same alongwith substance of the entire report including good points said in the praise of the officer
reported upon will be communicated to him/her in writing by the Reviewing Officer in his absence by the Reporting Officer
within a month of completion of the report in a narrative form so that the identity of any one of the assessing officers is not
disclosed (as per para 8.5 of Ministry of Home Affairs Cabinet Sectt. Deptt of Personnel O.M. No. 51/5/72/A Ests (A) dated
20.05.72). A copy of the communication letter and acknowledgement from the officer reported upon obtained as a token of
having received the communication letter will be sent to IHQ of MoD (Army), E-in-Cs Branch/MIS (Civ) for record.

Para 12 and 13 :This should be filled in brief and as far as possible in point form. Please fill up within the space
provided. No loose sheets should be attached. The reporting officer in the beginning of the reporting year (or on arrival of a
newly posted officer) will set quantitative/physical/financial targets in consultation with each of his officer on whom he is
required to report upon. The target should be understood by both the officers. Eight to ten targets need to be set and should
be given in order or priority.

(i) A separate Secret Note should be recorded and followed up. A copy of the note should also be sent together
with the Annual Performance Assessment Report to the next superior Officer who will ensure that the follow up action is
taken expeditiously. Where it is not possible either to certify the Integrity or to record the secret note, the Reporting
Officer should leave the space blank and state in Column 29 whether he had watched the officers work for sufficient time
to form a definite judgment or that he heard nothing against the officer, as the case may be.

(ii) If, as result of follow up action, the doubts or suspicions are cleared, the officers Integrity should be certified by
the countersigning officer and an entry made accordingly at the IHQ of MoD (Army) level in the Annual Performance
Assessment Report.

(iii) If the doubts or suspicions are confirmed, this should be duly communicated to the officer concerned by the
Countersigning Officer and the same be recorded in the officers APAR.

(iv) If as a result of the follow up action, the doubts or suspicions are neither cleared nor confirmed, the officers
conduct should be watched for a further period and thereafter action taken as indicated at (ii) or (iii) above.

grading

(a) / The columns in the APAR should be filled


with due care and attention and after devoting adequate time.

(b) It is expected that any grading of 1 or 2 (against work output or attributes or overall grade) would be adequately
justified in the pen picture by way of specific failures and similarly, any graded of 9 or 10 would be justified with respect to
specific accomplishments. Grade of 1-2 or 9-10 are expected to be rare occurrences and hence the need to justify them.
In awarding a numerical grade the reporting and reviewing authorities should rate the officer against a larger population of
his/her peers that may be currently working under them.

graded between 8 and 10 will be rated as outstanding and will be given a score of 9 for the purpose of calculating average
scores for empanelment/promotion.
