AFFIDAVIT OF HEIRSHIP

I, ELIZABETH JANAE BELLOSILLO BABAO, single, of legal age, Filipino and

resident of Brgy Libby, Puan, Davao City, Philippines, after having been sworn to
in accordance with law hereby depose and declare:

That I am the same persons who caused the execution of this affidavit;

That I am the biological daughter of spouses, Victorio S. Babao, Jr. and

Rhodora Bellosillo, who died intestate on September 22, 2010 and August 18,
2011, respectively, in Davao City;

That they left a parcel of land described as Lot15-E2, Psd-11-063284 with

an area of 1,000 square meters, more or less, situated at Brgy Libby, Puan,
Davao City, which they acquired from Avelina B Saavedra through donation;

That I am executing this affidavit to attest to the truth of the foregoing

facts and for my application for water and electrical services with Davao City
Water District and Davao Light and Power Company, respectively.
