Introduction
The Department of Health and Human Services has been working with health services to implement a
patient level data collection for specialist clinics in Victoria. Specialist clinics were introduced into the
Victorian Integrated Non-Admitted Health (VINAH) Minimum Dataset from 1 July 2012 following
extensive work on the part of health services and the Department to establish the necessary
infrastructure and reporting capability.
This work is ongoing and inherently challenging, particularly for health services with the largest volumes
of specialist clinic records to process and complex systems to modify. Implementation of VINAH has
therefore progressed to a variable extent across health services and across clinics within each health
service.
Consequently, this report presents data for only a subset of health services and specialties. Although the
health services included in this report have progressed well in implementing standardised data collection
for specialist clinics, activity and wait times reported here may be based on incomplete data and results
should be interpreted with caution. The completeness and accuracy of specialist clinic data will improve
over time.
It should be noted that a pilot audit program of specialist clinics data was undertaken at selected health
services in 201415. The Department of Health and Human Services is expected to commission an
independent audit program in 201516 to assess the integrity of VINAH data.
As VINAH data improve over time, this collection will enable public reporting of a range of activity and
performance data on specialist clinics in public hospitals, and will improve management through a better
understanding of this component of the system.
This report is based on preliminary data. Quarterly reports will be republished with refreshed data
following annual consolidation of the VINAH collection.

The VINAH Minimum Dataset


The Victorian Integrated Non-Admitted Health (VINAH) Minimum Dataset now integrates patient-level
data across many programs including Acute Non-Admitted (Specialist Clinic) Services, Sub Acute
Ambulatory Care Services, Hospital Admission Risk Program, Post-acute Care, Family Choice Program,
Victorian HIV Service, Victorian Respiratory Support Service, Community Palliative Care, Transition Care
Program, and Residential In-reach.
Currently, the scope of VINAH for specialist clinics includes services provided to non-admitted, non-
emergency department patients registered for care by 26 health services. All specialist clinic types are
included, except specialised mental health and alcohol and other drug treatment services.
The data elements collected in VINAH are standardised to those within the National Health Data
Dictionary (NHDD).

Definitions
Specialist Clinic
A specialist clinic is a specialty unit or organisational arrangement under which a hospital provides
specialist clinic services. The nature of the service provided by the clinic is classified by clinic type. All
clinics are reportable to the VINAH Minimum Dataset, except specialised mental health and alcohol and
other drug treatment services.

Specialty
The unit within a health service that is responsible for providing planned non-admitted services in an
acute setting. Health services assign the specialty at the time a referral is triaged and a specialist
appointment is booked for the patient.

Episode of Care
An episode or care is the period during which a patient receives services within a speciality. An episode
of care may include contacts reported as publicly funded by the Victorian Government and/or contained
within the Medicare Benefits Schedule (Medicare services subsidised by the Australian government). An
episode may include contacts across different specialties if each contact is linked to the same referral
record within VINAH.

First Appointment
A first appointment is the first patient consultation within an episode of care that has been initiated by a
referral from a General Practitioner or Specialist external to the health service, or the first scheduled
appointment if the patient failed to attend as planned. There can only be one first appointment per
episode.

New Consultation
There can be multiple new appointments per episode but only one new appointment per specialty within
the episode. In the event of multiple new consultations being attached to the same referral/specialty
only the first instance where the patient attends is counted as a new consultation. A new consultation
differs from a first appointment as the former may be initiated by a referral within the same health service
whereas the latter defines the patients first contact with a health service for an episode of care.

Review Consultation
A review consultation is an appointment attended by the patient for the primary purpose of reviewing the
patient following a previous outpatient appointment or treatment as an inpatient.

Median Wait Time


The median wait time represents the number of days waited by 50 per cent of patients who recorded
their first appointment within the quarter. Wait time is measured as the number of days between the time
a health service receives a referral for specialist services and the patients first appointment relating to
that episode.

90th Percentile Wait Time

The 90 percentile wait time represents the number of days waited by 90 per cent of patients who
recorded their first appointment within the quarter. Wait time is measured as the number of days between
the time a health service receives a referral for specialist services and the patients first appointment
relating to that episode.

Urgency
The clinical urgency of a patient referred for a specialist clinic appointment, recorded after the triage
process is completed. A referral is urgent if the patient has a condition that has the potential to
deteriorate quickly with significant consequences for health and quality of life if not managed promptly.
The clinically-recommended waiting time for urgent referrals is no more than 30 days of the receipt of the
referral. A referral is routine when a clinician determines that a patient does not need to be seen within
30 days of the receipt of the referral.

Explanatory notes to tables


Data limitations
This report presents data for only a subset of health services and specialties across Victoria. Activity and
wait times reported here are derived from an immature data collection and, consequently, results should
be interpreted with extreme caution.

Revisions
Data in the report are current as at 26 July 2016; however, variation to published figures is likely to occur
as health services continue to improve the quality of outpatient data. Revisions to quarterly data may be
published in subsequent reports.
