TERMS & CONDITIONS

1 Membership cards are the property of MW Restaurant and must be returned upon demand. A fee will be
assessed for replacement cards. Cards are non-transferable. Dining credits must be used with a year from date
of purchase of card. More credits may be purchased throughout the year if needed. Cards are renewable every
year.

2 Valet gratuity not included

3 Service fee breakdown; Receipt to be emailed within 24 hours of service. 4% of the service fee is allocated to
administrative costs. The remaining 18% of the service fee is allocated to the waitstaff.

4 Waived corkage fee equivalent to $25/bottle

5 Booking period to be opened at least 30 days prior to event or holiday and will be open for priority reservations
up to 7 days prior to event or holiday.

6 Discount applied only to Artizen by MW catering orders and not dine-in.

7 A maximum of 20 membership cards per account is allowed per corporate account.
