1. Reference: PD 1612 (Anti-Fencing Law).

2. On July 18, 2017 at about 2:30 in the afternoon the undersigned

together with SP02 Jonathan A Ogano and SP02 Jay P Amtalao conducted Visitorial
Power to Japong Auto Parts owned by Krizler L. Tionson and managed by Mr. Jaime
LLeano located at National Highway, Sta Rosa, Bayombong, Nueva Vizcaya.

3. The owner/proprietor of the said establishment presented their Mayors

Permit and DTI certifications as a proof of legality of their business.

4. All inspected spare parts were covered with receipt of their source

2. Reference: MV/MC Maintenance and Utilization Record

2. On July 14, 2017 at about 1:30 in the afternoon the undersigned

brought the Mobile car at Solano, Petron Gasoline Station for Change oil.

3. On July 15, 2017 same was brought to repair shop also in Solano this
Province for the repair and repainting of some minor scratches.

4. On July 20, 2017 at about 3:30PM mobile was car washed at

Bayombong, Nueva Vizcaya.
