1, Sampao, Isulan, Sultan Kudarat, after having been duly sworn to in accordance with
law depose and say:

That I am the father of STEPHANIE AQUINO REPOLLO, who was born on

September 7, 2000 at Laguilayan , Isulan, Sultan Kudarat.

That I acknowledged STEPHANIE AQUINO REPOLLO, as my daughter.
