AFFIDAVIT OF CONSENT TO TRAVEL

I, MOISES BLAGO, of legal age, Filipino, widower and a resident of New Tarlac,
Esperanza, Sultan Kudarat, Philippines, after having been duly sworn to in accordance
with law, depose and say:
1. That I am the biological father of ANGELIKA GRACE LAPADA BLAGO,
NOREEN BERNADETH LAPADA BLAGO and JASPER BRYAN LAPADA
BLAGO, attached herewith are the machine copies of their Birth Certificates
of Live birth;

2. That I am giving my full consent to my said children to travel abroad or

specifically to allow them to join a group family tour in Singapore and
Malaysia this coming April 23, 2017 to April 27, 2017 with their aunt,
VENUS L. GESTUPA;

3. That I am executing this affidavit to give my consent to the travel of my

children and to show proof that after their tour they will return to the
Philippines.

4. That I am executing this affidavit attesting to the truth of the foregoing

statements and as a supporting document for their Singapore application and
for whatever legal purpose this may serve best.

That I am the aunt of ANGELIKA GRACE LAPADA BLAGO, NOREEN

BERNADETH LAPADA BLAGO and JASPER BRYAN LAPADA BLAGO, all minors,
their late mother, being my sister, ;

That our family are planning to have a vacation/tour in Singapore and Malaysia
this coming April 23, 2017 to April 27, 2017 and I decided to bring along my said
nephew and nieces;

That I am the one who will shoulder all their personal expenses and allowances
incurred during our tour including their transportation expenses and travel documents;

That I am capable financially of supporting their travel expenses;

That I am executing this affidavit attesting to the truth of the foregoing statements
and for whatever legal purpose this may serve best.

5. That I am the one who manages the LAPADA ENTERPRISES owned by my
parents ANGEL & NORINA LAPADA dealing with buy and sell of
agricultural products with business address along the National Highway, New
Tarlac, Esperanza, Sultan Kudarat, machine copy of Business Permit No.
2013-036 issued by the Office of the Municipal Mayor of Esperanza, Sultan
Kudarat is hereto attached;

6. That I am the one who is managing said business for the reason that our
parents are already old, our father being 79 years of age and our mother 71
years old, attached herewith is their Certificate of Marriage;

7. That together with my sisters and their family, we are planning to have a
group family tour in Korea this coming December 22 to 28, 2013 to see the
beautiful scenery of Korea;

8. That I hereby declare that I have sufficient and adequate financial capacity to
support and defray all my expenses on said tour/travel and to guarantee that I
will not be a burden to the country or to any destination at any time during
my stay abroad;

9. That I am executing this affidavit to show proof that after our tour I will
return to the Philippines, my place of residence and where my source of
income and properties are.

10. That I am executing this affidavit attesting to the truth of the foregoing
statements and as a supporting document for my Korean visa application and
for whatever legal purpose this may serve best.

Esperanza, Sultan Kudarat is hereto attached;

12. That since we can no longer tackle the demand of our business due to old age
we decided to let our daughter, REGINA C. LAPADA to manage it

13. That our said daughter is planning together with his sisters a group family
tour in Korea this coming December 22 to 28, 2013.

14. That we are executing this affidavit to give our consent and support to the
travel of our daughter and to show proof that after their tour she will return
to the Philippines for she is the one taking care of us and managing our
source of income and properties.

15. That we are executing this affidavit attesting to the truth of the foregoing
statements and as a supporting document for her Korean visa application and
for whatever legal purpose this may serve best.
