2017 Year Two Clinical Skills - Formative Assessment

OSCE ASSIGNMENT
This assignment is designed to enable you to get the most out of the consultation you recorded with the
actor. By analysing what you did well, and what you did not do so well, you can use the information you
gather from this process to build on your strengths and to remedy any areas of weakness.

Your own tutor will mark your recording and this assignment. Together with the assessment of your
performance on the day of the formative OSCE, these three components help us to give you feedback
on how well you are doing with the Consultation Skills component of the course. This assignment should
take you about two hours to complete, and should be submitted electronically through Moodle. Please
do not submit by email directly to your tutor.

Please check that you complete all the following steps before your tutorial in Week 17:

1. Record Formative OSCE consultation (Week 16 tutorial)


2. Share your recording with your tutor on the Bracken website https://vbr.brackenlearning.com/
3. View your recording and answer the questions in this assignment
4. Submit your draft assignment to SafeAssign (optional)
5. Submit the final version of your assignment to Moodle

The instructions for sharing your recording with your tutor are in your Clinical Skills workbook, p 102.

Please note that we will run all assignments through SafeAssign after submission and you have the
opportunity to submit a draft through SafeAssign to check your work before submission. Go to

questions please identify the question you are answering by number only (e.g. 1a), do not re-
write the question or it will cause problems in SafeAssign.

Part One (~45 minutes)

Watch your consultation closely and make a note of points in the consultation which you thought went
well, and times when you thought you could have done things differently.

1a. Transcribe TWO such segments:

One segment where you think things went well


One segment where you think you could have done things differently

Use P for the patient and the first initial of your first name for yourself.

Transcribe it word for word, and include pauses, ums and ahs, laughter etc. If there is an interruption
note it as follows:

Explain what you think you did well OR what you think you would do differently if you had
the opportunity to re-run the consultation.

Part Two (~60-75minutes)

Analyse your interview and respond to each of the following tasks. (You may find that you need to replay
segments to do this adequately).

1. The GP would like you to formally present the patient to them. Please write down what you
would say.

2. Did you adequately explore the presenting problems from a biomedical perspective? (symptom
analysis, sequence of events, relevant system review and patient context)
What communication skills did you use?
What other information would have been important to gather, to fully explore the
biomedical perspective?

3. Did you adequately explore the presenting problem from the patients perspective? (ideas,
concerns, expectations, effects)
What communication skills helped you to do this?
What other information would have been important to gather, to fully explore the
patients perspective?

4. Did you follow "open-to-closed cones"?


a) Give an example
b) Give an example of a closed question that was effective
c) Give an example of a closed question that didnt work, and rephrase it into an
open question
d) Are you able to identify a leading question? Please give an example. What
effect did this have on the consultation?

5. Did your consultation have a discernible structure?

If so, how did you achieve this?

6. The GP would like to know your differential diagnosis. Please list at least THREE, from more
likely to less likely.

7. Why did you choose these diagnoses AND what lead you to put them in this order?

8. Using relevant examination frameworks, bullet point your approach to a thorough examination of
this patient, with brief explanations for each specific examination element listed.

9. Having reflected on your Formative OSCE, what do you want to keep working on? Describe
TWO things.
