JOINT AFFIDAVIT OF LEGITIMATION

WE, SPS. Alice S. Lost, and Lucas S. Lost, of legal ages, married, Filipinos and
residents of middle of nowhere, after having been duly sworn to in accordance with law,
hereby depose and say that;

1. We are the biological parents of MaryMary S. Lost, II, who was born on
November 1, 2006 at MON;

2. When MaryMary S. Lost, II was born she was considered ILLEGITIMATE

because we were not legally married then, although We did not possess any
disqualification to marry;

3. Subsequent to our marriage, which was celebrated sometime on August 6, 2007 at

Office of the Municipal Mayor, at guess where, we now request for the
legitimation of our said daughter before the Local Civil Registry of guess again.
Copy of our Marriage Certificate is hereto attached and made an integral part
hereof; and

4. We are executing this affidavit to attest to the truth of the foregoing for the
information of proper authorities concerned and for whatever legal intents and
purposes it may serve.
