and its registration covered by TCT Nos. T-100001, T-100002 &
T-100003, all situated in Binangonan, Rizal, respectively,
(attached copy of Plan and Title).

RESOLVED FINALLY that the corporation has

authorized its President and CEO, Mr. Basa to sign for and on
behalf of the Corporation in the foregoing.

The undersigned further certified that the foregoing resolution has not been amended,
revoked or otherwise modified and remains valid and subsisting.

1. That, I am the Heir Representative of the Hrs. of Guido who is the owner of
a parcels of land situated in Binangonan, Rizal registered under TCT Nos. T-
100001, T-100002 & T-100003;

2. That said properties were subdivided under plans Psd-04-150143, Psd-04-

150144 & Psd-04-150145 on July 20, 2002 executed by Engr. Fernandez
and was approved on May 2, 2002 in favor of the Hrs. of Guido;

3. That the certified copies of the approved plans together with the
corresponding narrative technical description, we found out that the folder
containing the said plans were missing;

4. That we exerted diligent efforts to locate or retrieve the folder containing the
approved plans (SEPIA) and other documents, it could no longer be found
and are considered lost;

5. That the said properties were already sold to the Goldland Properties and
Development Corporation;

6. That I am executing this affidavit to establish the circumstances surrounding

the loss of our plans Psd-04-150143, Psd-04-150144 and Psd-04-150145
copies and for the purpose of securing a new certified SEPIA copies and
narrative technical descriptions in lieu of the lost one.

This is in connection with approved plans Psd-04-150143, Psd-04-150144 & Psd-

04-150145 surveyed on July 2, 2002 executed by Engr. Fernandez and was approved on
May 2, 2002 of a parcel of land Lots 1, 6 & 10, (LRC) Psd-240150 covered by TCT Nos.
T-100001, T-100002 & T-100003 all registered to the Hrs. of Guido, may we have the
honor to request for a certified SEPIA copies and technical descriptions of said plans for
registration.

The said properties were already transferred to the Goldland Properties and
Development Corporation and were registered under TCT Nos. T-100004, T-100005 & T-
100006;

The copy of approved plans Psd-04-150143, Psd-04-150144 & Psd-04-150145

(SEPIA) together with the narrative technical descriptions were lost as stated in the
affidavit of loss executed by the Hrs. of Guido;

Further, the said plans has not been annotated nor registered in the Registry of
Deeds of Binangonan, Province of Rizal.
